Description
This evaluation focuses on the delivery of GP and nursing care to people sleeping rough and listed on the By-Name List in the Port Adelaide Enfield region. The evaluation will seek, where possible, to:
- Assess whether the model delivers dependable and continuous access to primary healthcare
- Capture the clinical and psychosocial complexity of the service cohort
- Identify service gaps, system barriers, and structural constraints to holistic care
- Highlight the value of sustained, relational GP and nursing care for people sleeping rough
