The utility of the ‘consideRATE’ questions to measure patient experience of palliative care at Sacred Heart Health Services: A quality improvement project

To determine, prior to discharge from the Sacred Heart Health Services (Sacred Heart) palliative care unit (PCU):
1. Whether >70% participation + >80% full completion of consideRATE by patients or their caregivers is achieved.
2. How consideRATE is completed, including who is involved.
3. Patient, caregiver, and clinician feedback about the utility of consideRATE at that timepoint.
