    Abstract

    Background and Aims: A recently published RCT by our group demonstrated clinical benefits of a chronic disease management (CDM) model including non-significant trends of both improved hospital utilization (increased proportion of elective versus emergency admission) and reduced mortality (40% reduction). In this follow-up study, our aim was to assess the relative cost effectiveness of the CDM model when compared to usual care during the 12-month study period, using incremental costs per death avoided as the primary outcome.
