Ambulatory care delivery for chronic disease management

Abstract

We describe ambulatory care tools we have developed for application to elderly chronic disease patients, living at home and accessing local primary health care services. This project is an example of where improved expert support for decision making is provided, by improving access to evidence-based information at the point of care.
