Abstract
Up to 50% of smokers may develop clinically significant COPD. Despite dissemination of evidence-based guidelines, there is still under-diagnosis and under-management in Australian primary care. We describe the implementation and evaluation of an interdisciplinary model of care-RADICALS© (Review of Airway Dysfunction and Interdisciplinary Community-based care in Adult Long-term Smokers) for reducing the burden of smoking and COPD in patients attending general practices.
