Breakthrough fusariosis presenting initially with pyomyositis

Management of invasive fungal infections remains a challenge in practice of haematological malignancies undergoing high dose chemotherapy and allogeneic stem cell transplants. The presence of invasive fungal infections contributes to high mortality and morbidity after high dose chemotherapy and stem cell transplant. Fusarium species related invasive fungal infections have been increasingly isolated in the era of broad spectrum antifungal prophylaxis. We present a case of breakthrough disseminated fusariosis despite posaconazole prophylaxis in an immunocompromised patient whose initial presentation was bilateral lower limb pyomyositis.
