Abstract
Recent clusters of foodborne botulism in Italy – one linked to guacamole consumption in Sardinia, and a more lethal one in Calabria involving sandwiches containing oil-preserved vegetables – signal a disturbing recurrence of a rare but devastating microbial hazard. These events, despite occurring within regulated commercial and semi-industrial food chains, underline the tenacity of Clostridium botulinum and demand renewed scientific and public health scrutiny. Botulinum neurotoxins (BoNTs) are among the world’s most potent biological poisons, with lethal doses in humans estimated at less than 1 ng/kg body weight. Foodborne botulism results from ingestion of preformed toxins in foods – often those with low acidity, anaerobic storage conditions, or inadequate thermal processing. Although rare, the clinical severity is marked – ranging from bulbar palsy to descending paralysis and respiratory arrest, necessitating mechanical ventilation and prompt administration of equine-derived heptavalent antitoxin.
