Abstract

The authors advise some of the tables and figures refer to persons when the numbers reflect person-episodes of care. This has been described in the Methods; however for this to be clearer the following text should have been added as a footnote to Tables 2, 3 and 4 and Figs 1 and 2:

All results are based on person-episodes of care.
