Abstract

The prevalence of atrial fibrillation (AF) is increasing rapidly and it is expected to rise to epidemic numbers in the near future. Current practice is challenged by these increasing numbers of AF patients with multiple health needs and is often struggling to adhere to evidence-based guidelines, which results in fragmented care.
