    Abstract

    Hospital admission is a process at the interface of care where inaccuracies in medication-related information can occur and the risk of medication errors and adverse drug events is substantial.1-5 We report a trial of a pharmacist service in the surgical preadmission clinic at Flinders Medical Centre from February to April 2010.
