Abstract

Aims:
We aim to describe the design of the Country Heart Attack Prevention (CHAP) Project, a telehealth-enabled model of care for cardiac rehabilitation (CR).

Methods:
Following the four stages of the Model for Large Scale Knowledge Translation over a decade of partnership, researchers, clinicians and health service managers summarised evidence on telehealth-based CR, identified local barriers to CR attendance, measured performance, and developed the CHAP model of care. This will be delivered to all eligible patients living across 6 local health networks in rural South Australia (SA) from July 2021. Impact on CR attendance/completion, clinical outcomes and cost-effectiveness will be tested through a pre-post and cohort
study over 12 months.
