Abstract

Our aims and measures of healthcare safety and quality and our ideas on improving them are framed within a Newtonian way of reasoning. That is, to understand complex they need to be reduced to a discrete collection of isolated phenomena parts making them into more or less meaningful or relevant categories. Here, errors are prevented by policies and procedures, audit, regulation and enforcement.
